Perimenopause brain fog at work: what is actually happening and what helps

Brain fog is one of the most debilitating perimenopause symptoms for professional women. Here is what the evidence says about why it happens and what nutrition can do about it.

Why perimenopause causes brain fog

Brain fog is not a vague or imagined symptom. It has a documented physiological mechanism — and understanding it is the first step to doing something about it.

Oestrogen plays a significant role in cognitive function. It supports the production of acetylcholine, a neurotransmitter involved in memory and attention. It also influences serotonin and dopamine pathways, which affect mood, motivation and mental clarity. As oestrogen fluctuates and eventually declines during perimenopause, these systems are disrupted.

The result is what most women describe as brain fog — difficulty with word retrieval, working memory lapses, problems with sustained attention, and a general sense that the mental sharpness they have always relied on is no longer consistently available.

For professional women, this is not a minor inconvenience. It lands directly on the capabilities that their careers depend on most.

What the research actually shows

A 2021 study published in Menopause found that women in perimenopause showed measurable declines in verbal memory and processing speed compared to premenopausal women. Importantly, the research also showed that these cognitive changes are often temporary — many women report improvement once hormonal fluctuation stabilises post-menopause.

That matters. Brain fog in perimenopause is a phase, not a permanent state. But it is a phase that can last several years, and for women in demanding careers, several years is a long time to operate below their cognitive baseline.

The sleep connection

Much of the cognitive impairment associated with perimenopause is mediated by sleep disruption. Vasomotor symptoms — night sweats and hot flushes — cause fragmented sleep, and sleep deprivation has well-documented effects on memory consolidation, attention, and executive function.

Anyone who has dealt with a toddler — or been sleep deprived for any other reason — knows what this feels like. The perimenopause version is the same mechanism, repeated night after night, often for years.

Addressing sleep quality is therefore not separate from addressing brain fog. They are the same problem.

What nutrition can do

The evidence on nutrition and cognitive function in perimenopause is still developing, but several areas are well supported.

B vitamins — particularly B6, B12 and folate — are involved in the synthesis of neurotransmitters including serotonin and dopamine. Deficiencies in B12, which become more common with age due to reduced absorption, are associated with cognitive impairment. Ensuring adequate intake through diet or supplementation is a reasonable and well-evidenced intervention.

Magnesium plays a role in nerve function and sleep quality. Magnesium deficiency is associated with poor sleep, anxiety and cognitive fatigue. The form matters significantly — magnesium bisglycinate is considerably more bioavailable than the magnesium oxide found in most supplements.

Creatine is perhaps the most underappreciated nutrient in this context. Research has consistently shown that creatine supplementation supports cognitive performance, particularly under conditions of stress or sleep deprivation. A 2022 study found that creatine supplementation improved working memory and processing speed in conditions of sleep deprivation — which describes the situation many perimenopausal women are in nightly.

Creatine is also produced endogenously from animal protein, which means women who eat less meat — or who are increasing plant-based eating as they age — may have lower baseline levels.

What does not help

The perimenopause supplement market is full of products making cognitive claims without the evidence to support them. Proprietary blends that do not disclose doses, ingredients at sub-therapeutic levels, and combinations with no plausible mechanism are widespread.

The honest position is this: nutrition can support cognitive function during perimenopause, but it is not a substitute for addressing the underlying hormonal changes. If your symptoms are significantly affecting your quality of life or your ability to work, speaking to a GP or menopause specialist about the full range of options — including HRT — is the right starting point.

Nutrition sits alongside that conversation, not instead of it.
